HALFTIME AND FINAL SCORE PREDICTION
CONDITIONAL PRIZE CONTEST

1. WZNS-FM will conduct the contest on the above listed contest date(s) for a chance to win the following grand prize:
a.) Halftime and Final Score Prediction Grand Prize Trip for Two to the Super Bowl or $10,000.00 Cash
2. Contest can be entered prior to game-time at Helen Back, 114 Amberjack Drive, Fort Walton Beach, Florida every Monday night during the NFL Monday Night Football 2008 Regular Season. No Purchase Necessary. Each contestant must fill out an official entry blank for the chance to become an official finalist in the conditional grand prize contest giveaway. All entrants must attempt to predict the exact halftime and final score of the designated NFL Monday Night Football game. The exact halftime and final score prediction must be clearly and legibly recorded on the official entry blank to be considered eligible to become an official finalist prediction. Only one entry per person per game will be accepted.
3. A total of 1 finalist per game will be randomly selected from all the entries received for the chance to win the grand prize
4. At the completion of the designated game, the 1 official prediction(s) will be compared against the actual halftime and final score. A Grand Prize winner will be declared only if the 1 official prediction(s) exactly matches the actual halftime and final score of the designated game. Maximum (1) grand prize winner. If none of the official finalist(s) prediction(s) matches the actual halftime and final score of the designated game, no grand prize will be awarded.
5. If there is a grand prize winner, the winner must come to WZNS-FM at a mutually agreed date and time at least two weeks prior to the delivery of the grand prize.
6. The contest is open to persons 21 years of age and older who are legal U.S. residents with a valid driver’s license. Employees of WZNS-FM, Helen Back, Miller Lite, other related sponsors, and their immediate families may not participate and are not eligible to win. The term “immediate families” includes spouses, grandparents, parents, siblings, children, and grandchildren.
7. In addition, all score prediction entries from each week will be retained for a grand prize random drawing to be held on Thursday, December 4, 2008, for a VIP trip for two to the 2009 Pro Bowl in Hawaii, compliments of Coors Light, the Official Sponsor of the NFL. The trip will include round trip air fare for two, hotel accommodations, two tickets to the Pro Bowl and passes to the Coors Light/NFL VIP party. In some circumstances, the 2009 Pro Bowl trip drawing may be held one week before or one week after December 4, 2008.
8. Persons who have won another contest or promotion of any kind from WZNS-FM within 30 calendar days before the contest date are ineligible to win. Any person who has won a prize/prizes valued at over $600 on any Cumulus station within the past year will not be eligible.
9. Difficulties with getting to locations to enter the contest are not the responsibility of WZNS-FM.
10. As winner of the grand prize, you will be required to sign a release form before your prize will be awarded. You will be responsible for all-applicable registration fees, title fees, and taxes including state, local, and federal and therefore must present a valid Social Security Card before claiming your prize.
11. As a grand prize winner, you acknowledge that WZNS-FM and American Hole ‘n One have the right to, without further authorization, publicize your name, character likeness, photograph, voice, and the fact that you are a grand prize winner for promotional purposes without financial remuneration.
12. There is no purchase necessary to enter or win the contest. Void where prohibited.
13. There will be no substitution or cash alternatives for prizes. Participation in the contest and the prizes awarded are non-transferable. Failure to collect the prize constitutes forfeiture of the prize. If forfeited, the prize will remain the property of WZNS-FM. Another winner will not be selected.
14. WZNS-FM reserves the right to make rule or contest changes without notice at any time. WZNS-FM also reserves the right to change the aforementioned dates without notice. WZNS-FM reserves the right to end a contest at any time upon advanced notice to the public.
15. The decision of WZNS-FM management in the interpretation of these rules is final. By participating, persons agree to be bound by the decisions of the judges and the official contest rules.
16. If the winner is found in violation of the rules, he/she may be required to forfeit his/her prize.
17. WZNS-FM, their representatives, agencies and employees, are not responsible for any liabilities arising directly or indirectly from the award or use of the prize. All entrants and winners agree that WZNS-FM, Cumulus Broadcasting LLC, their affiliates, and sponsors shall have no liability and shall be held blameless for any for any injury, misfortune, or damage to either persons or property insured by entering, participating in, winning or losing any contest by use or non-use of any prize received.
18. WZNS-FM, their representatives, agencies and employees, are not responsible for any changes or difficulties in travel or prize delivery.
19. All entry blanks, forms, devices and materials, as well as all the information contained on or within, gathered during the course of entry, shall become the sole property of the station to be used, disposed of or destroyed at the discretion of the station. Such use including, but not limited to, enrolling entrant in one of more of the station’s listener clubs and sending entrant correspondence and solicitations with regard to such clubs. Further Cumulus Broadcasting LLC is not responsible for lost, mutilated, illegible, misdirected, late, stolen, incorrect or incomplete entries. WZNS-FM and Cumulus Broadcasting are not responsible for any incorrect or inaccurate information entered by website sers, and assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to the website.
20. The chance to win the grand prize includes:
1 finalist(s) will be randomly selected prior to the start of the designated game for the chance to win. At the completion of the designated game, if any of the official finalist prediction(s) exactly matches the actual halftime and final score for both teams, that finalist will win the grand prize, subject to verification. The prize does not cover any other expenses, including but not limited to tax. WZNS-FM is not responsible and provide no warranties regarding the operation or use of the grand prize. Other restrictions may apply.
